Aging Network Program offered at Area Agency on Aging and Disabilities of Southwest Washington

Assists clients in exercising their options for community-based care. Services provided through this program are designed to ensure that clients experiencing complex problems and/or disabilities receive needed services in a timely and appropriate fashion, and prevent unnecessary institutionalization. Services may include case management, as well as personal care services.

Federally designated agency on aging for Clark, Cowlitz, Klickitat, Skamania, and Wahkiakum Counties. Promotes independence, choice, wellbeing, and dignity of people age 60 and older and adults with disabilities and their families through a comprehensive, coordinated system of home and community-based services.
