PACE (Program of All-inclusive Care for the Elderly) offered by International Community Health Services at Legacy House

Provides individualized services to keep nursing-home eligible individuals living independently at home or in the community as long as possible. A team of eldercare experts does an assessment, designs a comprehensive personalized care plan, then provides and coordinates services to address health and personal care needs. PACE health services include the following as needed: - Adult day services - Medical care - Caregiver support - Transportation to the facility - In-home care - Medication Management

Application process

Call to schedule an appointment with an enrollment specialist. An interdisciplinary team will review the patient's medical history to help determine if PACE is a good fit.

Fee

Accepts Medicare, Medicaid and private pay.

Age 55 or older In need of nursing home level of care as defined by the state of Washington Residing in the PACE service area at the time of enrollment Able to live safely in the community with PACE services Medicaid-eligible and/or willing to pay private fees

International Community Health Services

Manages four clinics that offer affordable primary medical and dental care. Staff is multi-lingual in more than 50 languages and dialects. Provides immunizations, family planning, prenatal care, well-child checkups, sports physicals, behavioral health services, nutrition services, and STD testing and treatment. Offers complete eye exam services for adults and children, including prescriptions for glasses and referrals for specialty care at the International District location. Operates the Chinese Traditional Medicine Clinic, which uses acupuncture, moxibustion, herbal medicine and acupressure to treat and prevent diseases. In addition, Operates a Mobile Dental Clinic, traveling to different Seattle Public Schools to provide dental services to students. Also operates two school-based health centers to provide on-site primary care and behavioral health services to students. Assists students and their families in finding health insurance, community-based health and wellness services, and other community resources.
