Tribal Medical Clinics offered by Cowlitz Indian Tribe

Provides medical services, including physical examinations, family planning, well child exams, immunizations, chronic disease supervision, limited lab services, medication management, nutrition counseling, diabetes screening and management, health education, and women's health. Offers a holistic approach to health that promotes disease prevention, treatment, and education. A benefits counselor is available to assist tribal members in locating and applying for benefits, such as Social Security, veteran benefits, Medicaid, and prescription drug programs.

Eligibility

- Any enrolled American Indian/Alaskan Native (AI/AN) of a Federally Recognized Tribe - Any direct lineal descendant of a federally recognized AI/AN (parent must be enrolled) - Any state recognized Native American or First Nations Tribal Member with a billable insurance - Dependents/Spouse of an Enrolled Cowlitz Tribal Member with a billable insurance - Tribal Employee, their dependents, and/or their spouse with a billable insurance
