Medicaid In-Home Care offered at Aging and Long Term Care of Eastern Washington

Aging & Long Term Care helps older adults and adults with disabilities live at home independently through in-home care and case management services. We offer Case Management (CM) to assist older adults and adults living with disabilities set up a safe plan of care to maintain independence. Persons on CM receive help from a caregiver (for help with personal tasks such as bathing, transportation, light housekeeping, dressing, and more), receive annual assessments to determine the best pathway of care, and have access to other programs to help them remain safe and happy at home. Upon referral, a RN Consultant can also provide medical assessment, educate caregivers on client specific information, consults and coordinates with the client care's team (such as physicians, social workers, and others), and evaluates clients skin conditions, nutrition, hydration, disease process, response and tolerance of medication and more.

Funds in home case management/personal care, senior nutrition, Family Caregiver Support, other community services such as Minor Home Repair, Respite, Legal Assistance, Title V Senior Employment, Statewide Health Insurance Benefits Advisors (SHIBA), and provides eligibility screening for the WSDA Dental Outreach program for low-income seniors.
