Case Management offered at Patient Advocate Foundation

Offers assistance to patients with specific issues they are facing with their insurer, employer and/or creditor regarding insurance, job retention and/or debt crisis matters relative to their diagnosis of life threatening or debilitating diseases.   Professional case managers and attorneys specialize in mediation, negotiation, and arbitration to advocate on behalf of patients experiencing issues with access to care, job retention and debt crisis due to their illness.

Eligibility

Those that : - Have a confirmed diagnosis of a chronic disease, a life-threatening disease, or debilitating disease OR - Be seeking screening services related to symptoms or suspicion of a chronic, life-threatening or debilitating disease - Be in active treatment, had treatment within the past 6 months, or going into treatment in the next 60 days - Be a U. S. Citizen or Permanent Resident of the U. S. - Be receiving treatment at a facility in the United States or one of its US territories - Have income at or below 400% of FPL
