Youth Case Management offered at Southwest Youth & Family Services

Helps youth and families navigate the school and court systems, coordinates and connects them to needed services such as educational support, housing, mental health and substance use disorder treatment, employment, and positive youth development activities.   Case Managers also provide outreach and advocacy, as well as culturally and linguistically appropriate support services.   They create linkages for youth and families, maintain regular contact with youth, and monitor youth progress towards reaching individual service goals.

Eligibility

Families of at-risk youth ages 14 through 17 who reside in Seattle, South King County or attend school within the Seattle or Highline school districts.

Provides advocacy regarding immigration and cultural transition, medical services, housing, legal and education. Offers mental health counseling for children, teens and families. Provides case management for at-risk youth. Offers a high school credit retrieval program and GED preparation for youth. Offers home visiting literacy program. Offers site-based literacy, academic support and family advocacy at Highline area KCHA apartments.
