Chronic Care Management offered by Bailey-Boushay House

Helps prevent or delay the need for institutional care for people with HIV/AIDS by helping them maintain independence at home, and by providing daytime relief for their families or caregivers through an outpatient program. Services include: -  Health monitoring -  Medication adherence -  Physical and occupational therapy -  Recreational and social activities -  Counseling and support groups -  Hot meals - Hair Cuts - Laundry/Showers -  Nutritional counseling   Interpreter service available Clients are provided with disability bus passes when eligible.

Daily, 9am-5pm.

Application process

Call the admissions coordinator.   Referrals are accepted from clients, families, caregivers, physicians, social workers and other health professionals.

Fee

Vary according to services.   WA Apple Health (Medicaid), private pay and other third party payment is accepted. Limited free care may be available through the Hill-Burton Act.

Eligibility

Individuals must have a disability related to AIDS.

Bailey-Boushay House

Provides a skilled nursing facility for people with HIV/AIDS; physical rehab, long-term care and end-of life care for those with complex conditions including ALS, Huntington's disease and cancers. Outpatient program supports individuals with HIV/AIDS offering medication management, chemical dependency and mental health outreach services. Emergency Homeless Shelter and housing stability programs for those enrolled.
