Case Management offered at Area Agency on Aging Thurston County
Case Managers assess the temporary and permanent needs of older and disabled adults who are receiving Medicaid in-home personal care services to coordinate these services in order to maintain maximum independence. Case Managers: - Assess needs of individuals on State-funded Medicaid personal care services, - Develop care plans, - Authorize and arrange services, - Coordinate provision of services among providers, and - Follow-up with re-assessments as needed.

Plans, coordinates and advocates for services for people ages 60 and older, disabled adults and unpaid family caregivers. Direct services provided include: - Information and assistance/referral, - Medicaid Case Management, - Family Caregiver Support Program, - Home Care Referral Registry (for those receiving Medicaid in-home personal care services), - Assisted listening system (ALS) available for community events. This is a conference/meeting equipment system designed to assist with the hearing impaired. Indirect services provided by contracted organizations include: - Adult day care, - Congregate meal sites, - Home delivered meals, - Transportation referrals, - Legal services, - Long-Term Care Ombudsman program, - Homecare services, - Personal Emergency Response Systems (PERS) - Respite for unpaid family caregivers, - Family Caregiver Counseling - Senior Farmers Market Nutrition Program (SFMNP), and - Kinship and family caregiver programs - Medicaid Waiver, RCL.
