CYSHCN - Children and Youth with Special Health Care Needs offered at Kitsap Public Health District

CYSHCN serves children who have or are at risk of having a serious physical, developmental, behavioral or emotional condition and require additional support. CYSHCN promotes integrated systems of care that assure children with special health care needs have the opportunity to achieve the healthiest life possible and develop to their fullest potential. Services can include: helping a family find or coordinate medical providers or a medical home, help accessing local resources or services for the child and provide information about local community resources, including parent support organizations and community coalitions.

Eligibility

Children 18 and under and their families in Kitsap County who are at risk for or have a health and/or developmental condition that requires more than usual care regardless of income.

Kitsap Public Health District has protected the health of our county’s residents since 1947. The mission of KPHD is to prevent disease and protect and promote the health of all people in Kitsap County. A 10-member Public Health Board, composed of local elected officials and community representatives, governs the work of more than 120 professionals in community health, environmental health, public health infrastructure, and administration.
